Case Manager (RN/Registered Nurse)

Position Summary:

Provides professional nursing care visits to patients in their home. Collaborates with the patient's case manager and other members of the patient care team. Independently provides and documents patient, visits. Assists with inservices and training as assigned. Must be literate in Word/Word Perfect or related software.


Licensed as a Registered Professional Nurse in the State of Ohio. Graduate of an accredited school of nursing. Must possess a minimum of two years nursing experience.

Key Responsibilities and Duties:

  • Supervises and is responsible for PCA's, HHA's, HMK's. ILA's in the field and provides appropriate documentation.
  • Functional operation of the clinical documentation program.
  • Provides home care nursing visits as assigned.
  • Participates in the quality improvement and utilization review activities of the organization.
  • Submits completed documentation of the home care visit within the established time parameters.
  • Attends multidisciplinary patient care conferences as requested.
  • Attends staff meetings and inservices.
  • Instructs patients and caregivers according to the plan of care.
  • Admits patients, develops and updates patient plan of care adhering to the Medicaid/Medicare criteria for home care provision.
  • Makes nursing assessments and reports significant changes in the patient's condition to the appropriate team members.
  • Communicates to physician any changes in the patient's condition.
  • Responsible for complete and accurate data collection.
  • Obtains physician orders for changing patient care needs.
  • Revises the plan of care based upon assessment findings.
  • Demonstrates a high degree of commitment to customer service and quality care provision.
  • Exhibits good written and verbal communications.
  • Refers patients to community resources as appropriate.
  • Maintains a list of clients and their schedules.
  • Maintains accurate and current drug profiles for patients as assigned using the documentation system according to home health agency policies.
  • Recommends discharge of patient's as appropriate.
  • Must provide on call coverage as appropriate.
  • Must possess a good driving record with no more than 2 traffic violations within the past 24 months.
  • Must possess a valid Ohio Driver's License.
  • Must have proof of comprehensive auto insurance coverage.
  • Performs other duties as assigned.

Immediate Supervisor:

Reports to the RN Supervisor.


At three months initially and annually thereafter by the RN Supervisor.
