Mental Health Case Worker (CPST Provider)

Connections:

Health Wellness Advocacy, a comprehensive community mental health organization, has immediate openings for Mental Health Case Managers in multiple specialty programs at our east and west side locations.

Responsibilities:

Provide a wide range of direct and indirect Mental Health Case Manager services to consumers with severe and persistent mental illness, including: assessment of strengths, abilities, needs, and preferences; referrals and linkage to resources; advocacy; coordination of serv

This position provides a wide range of direct and indirect substance abuse case management services to consumers with severe and persistent mental illness and substance use disorders. This position is community based and responsible for managing an assigned caseload of clients. Duties will include assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs. Case Managers' promote a recovery-based model for treatment of clients that is reflective of the agency mission statement.

Qualifications:

  • CDCA preferred

  • Bachelor's degree in social work, psychology, human services or related field required.

  • A valid Ohio driver's license and an insured vehicle.

  • Previous experience as a Mental Health Case Manager preferred.

  • Knowledge of benefits and entitlements, such as SSI/SSD, Medicaid, and Medicare eligibility preferred.

  • Bilingual in Spanish and English a plus!
