Position Summary

Licensed Clinical Social Worker (LCSW) is sought for Aging and Dementia outpatient group.

Responsibilities

* Support clinical practice and research patients.
* Provide education and outreach support for people with memory complaints and other disorders of the aging brain. Disorders include Alzheimer's disease, Lewy Body Disease, Frontotemporal Dementia, and other less common disorders affecting memory, thinking and requiring social support services.
* Engage in some research outreach activities.
* Assist patients and caregivers in obtaining social services.
* Complete forms required for referrals including M11Qs.
* Provide counseling and supportive consultations and treatment plans.
* Participate in staff meetings, in-services, and administrative activities.
* Give trainings and presentations regarding social services for aging population.
* Perform scales assessing mood and behavior and caregiver burden (such as the NPI-Q and GOS) on research participants and their

caregivers.
* Attend meetings and planning sessions in research activities.

Minimum Qualifications

* Must hold a Master Degree in Social Work from a CSWE accredited university/college.
* Licensed Clinical Social Work (LCSW) certificate from the New York State Education Department.

Preferred Qualifications

* Bilingual in English/Spanish strongly preferred.
* Three years of related experience.
