Job Description

CARE MANAGER

Health Homes Program. Full-time, schedule is Monday through Friday, somewhat flexible hours.

Minimum Education/Experience Requirements: Bachelor’s with 2 years of related experience or Master’s with 1 year related experience

Job Duties: Formal and informal assessment of strengths and needs of a child/family. Organize the team who can help meet those needs. Connect the child/family to needed services/resources. Fill in the gaps where support is needed but not available any other way. Create Plans of Care as well as emergency and safety/crisis plans. Coordinate the overall care of the child. Partnering with families, schools, doctors, mental health services, and other community services for the promotion of health and wellness.
