Schedule: This is a Per Requested Need (PRN) or ‘as needed’ position and there are not a guaranteed number of hours offered each week. This position is not benefits eligible.
Purpose: The Case Manager facilitates the care plan for the inpatient/Observation episodes of care, proactively monitor clinical quality and facilitate timely discharge. Using a collaborative interdisciplinary approach, the Case Manager will confirm the plan of care and focus on anticipation, integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients in order to optimize clinical outcomes and resource utilization. Utilization review and monitoring will be done on all payers.
- Current Colorado /RN license, BSN Preferred
- Current BLS from the American Heart Association
- 4 years of clinical experience as a Registered Nurse.
- 1 to 2 years experience in Case Mgmt., Utilization review and discharge planning in hospital setting.
- Knowledge of community resources used for discharge planning, hospital operations, excellent communication skills, knowledge of third party payment systems, Medicare/Medicaid programs.
- Ability to multi-task, set priorities and maintain organization.
- Needs basic computer skills.
Light work - (Exert up to 20lbs force occasionally, and/or up to 10lbs frequently)
