Job Summary
Provides professional nursing care to long term care residents in accordance with facility policies and procedures, under the supervision of the Nurse Manager and Nursing Supervisor.

Essential Functions

 

 

  • Administers medications, treatment therapy and procedures. Documents actions, follow up and resident response.
  • Performs charge nurse or team leader duties as necessary making rounds to determine level of standards of resident care, environmental safety and delegated tasks of CNA I and CNA II.
  • Reports resident care conditions and concerns accurately in the shift report to supervisor, family and/or physician.
  • Documents all necessary information on resident's record. Provides direct resident care, including all activities of daily living, answering call bells.
  • Knowledgeable of drug actions, administration, side effects and correct dosage.
  • Utilizes Sardis' policy and procedure manuals effectively for reference in dealing with problems.
  • Assist with admissions and discharges. Assist physicians as needed with rounds or procedures.
  • Transcribe and implement physician orders according to policy.
  • Reports incident and medication errors promptly via incident and medication error reports.
  • Demonstrate knowledge of IV's and skills of accuchecks and obtaining specimens.
Physical Requirements
Work requires sitting, lifting, reaching, stooping, bending, pushing and pulling. Must be able to lift and support the weight of 51 pounds in handling patients, medical equipment, and supplies. Must speak fluent English. Intact sense of sight, hearing, smell and touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient and/or service area conditions. Hours are occasionally long or irregular. Work environment is indoors.

Education, Experience and Certifications
Graduate of an accredited licensed practical nurse program. Experience in home health and/or acute care preferred. Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.
