Job Description

Position Summary:

Responsible for the delivery of primary health care with a focus on individuals who are homeless, living in public housing, or uninsured; function as primary care nurse


  • Experience as a community oriented nurse working with underserved populations preferred.
  • Familiarity with health issues including, but not limited to chronic illnesses, such as diabetes, hypertension and asthma, communicable diseases, HIV/AIDS, substance abuse, and mental illness.
  • Ability to work independently within scope of nursing practice with little supervision.
  • Ability to communicate and work with diverse populations, including clients, staff, students, volunteers, and the community.
  • Ability to manage multiple tasks and produce expected results.
  • Good leadership, interpersonal, team-building and communications skills.
  • Commitment to the mission of Care Alliance.
  • Valid driver's license, insurance and transportation


  • Triage patients with medical complaints and concerns and refer to provider as appropriate
  • Perform intake on patients according to Care Alliance intake procedures
  • Perform basic health assessments and screening
  • Perform basic procedures, including venipunctures, injections, EKG's, and assisting with other procedures.
  • Maintain accurate health care records.
  • Work collaboratively with other Care Alliance health professionals to facilitate a team approach to patient management
  • Provide education related to specialized health care needs of individuals who are homeless to service providers with whom Care Alliance works
  • Maintain adequate medications and medical supplies
  • Facilitates a safe and healthy environment in Care Alliance clinics
  • Conduct Nurse Clinics
  • Participate in community outreach, such as immunizations and health education as needed
  • Participate in community education and outreach activities, such as the Homeless Stand Down, community education classes and health fairs as appropriate
  • Participate in community activities, such as the Public Housing Advisory Board (Stokes and Riverview), Homeless Advisory Board, and NEOCH (Northeast Ohio Coalition for the Homeless) as appropriate
  • Assist with the development of nursing protocols
  • Assist with the orientation of new clinical personnel and volunteers
  • Supervise/coordinate students and volunteers
  • Performs other duties as assigned.


  • Registered nurse with current Ohio licensure; BSN preferred
  • Minimum three years nursing experience
  • Computer literacy required
  • Experience as a provider of clinical services; primary care, chronic care, outpatient, community health and/or psychiatric nursing background preferred
  • Previous experience with individuals who are homeless or living in public housing preferred

Typical Work Hours

Full time, 40 hours, flexible schedule (including evenings and weekends)
