Job Description

Position Summary:

Responsible for the delivery of primary health care to people in need in the homeless community; oversight in conjunction with the Outreach Operations Manager.

Essential Skills & Experience:

Registered nurse with current Ohio licensure (BSN preferred). 3 years RN experience, preferably in Outreach setting. Primary care, chronic care, outpatient, substance use and/or psychiatric nursing background. Previous experience with individuals who are homeless, low income, or living in public housing. Health teaching and experience and familiarity with community resources helpful.


Works as the liaison between Care Alliance and shelters and other places where individuals who are homeless stay and congregate. Performs intake, health assessments and screenings, referring to a physician or secondary/tertiary care provider as indicated. Provides direct care and encourages continued care as appropriate within Care Alliance clinics and/or other providers as appropriate. Maintains accurate health care records in Electronic Medical Records (EPIC) system. Maintains adequate medical supplies for outreach activities. Works collaboratively with other Care Alliance health professionals to facilitate a team approach to the management of its patients. Works collaboratively with community service providers, especially with other outreach workers. Identifies and links clients with available community resources. Provides education related to specialized health care needs of individuals who are homeless to service providers with whom Care Alliance works. Advocates for patients who are homeless and coordinates their health care. Coordinates the Homeless Client Advisory Board. Assists in Outreach Program development, implementation and evaluation. May provide supervision to health care outreach volunteers and students. Valid Ohio driver's license, insurance and transportation.
