Job Description

Position Summary:

Working with Care Alliance Health Center as part of the Patient-Centered Medical Home team, the Integrated Care Project Manager (ICPM) addresses and reduces barriers to wellness by approaching social, environmental, physical health, chronic disease, mental health and substance use. The ICPM is responsible for implementation of grant funded programming designed to move Care Alliance Health Center from a Level 4 to a Level 6 integrated care team (SAMHSA Integrated Care model framework). The ICPM will facilitate all aspects of this transition, including a comprehensive training program as well as the development and support of integrated teams. The ICPM will additionally be responsible for the development and implementation of a comprehensive evaluation protocol. The ICPM is a grant funded position through calendar year 2018.

Essential Skills & Experience:

Experience working with vulnerable populations (adults and children) with chronic illnesses, mental illness, and substance abuse; individuals who are homeless or living in public housing. Familiar with various healthcare and behavioral health care models and concepts, including the general disease model and stages of change. Competency in working with a team to provide components of healthcare conceptualization, consultation, treatment planning and motivational interviewing for treatment compliance. Working knowledge of integrated care models is required and familiarity with Electronic Health Record systems preferred.

Responsibilities/Requirements:

Master's Degree in applicable field, or Baccalaureate Degree with relevant experience in integrated care. Computer literacy required. Develop strategic implementation plan to move Care Alliance Health Center from a Level 4 to Level 6 integration. Develop protocol related to integrated care processes using the Plan, Do, Study, Act model. Working with organizational partners, coordinate and facilitate organization-wide training in Team STEPPS, motivational interviewing (MI) and Nonviolent Crisis Intervention (NVCI). Valid driver's license, insurance, and reliable transportation. Commitment to the mission of Care Alliance Health Center.
