Job Description

Position Summary:

Responsible for the delivery of primary health care with a focus on individuals who are underserved, including homeless, living in public housing, or uninsured. Function as charge nurse for selected clinics and based on Program Specifics.

Program Specific Duties:

Work with Care Alliance, shelter, and other community programs to identify, schedule and market classes and recruit participants. Form collaborative relationships with community health and social service agencies. Identify/create curricula and conduct educational sessions on women's health, including breast health, at area shelters and other community programs. Maintain accurate records of all education and outreach activities. Provide patient navigator services to homeless women and women in public housing to help them obtain breast and other women's health services. Participate in program evaluation. Familiarity with women's health issues and health teaching and curricula development experience helpful. Familiarity with community resources helpful. Pediatric experience, working with children though teens.


Experience as a community-oriented nurse working with underserved populations. Leadership experience to run a fast pace clinic. Excellent clinical skills; primary care, chronic care, outpatient, community health and/or psychiatric nursing background preferred. Familiarity with health issues including chronic illnesses, communicable diseases, HIV/AIDS, substance abuse, and mental illness. Ability to work independently within scope of nursing practice. Strong computer skills; experience with electronic medical records. Excellent oral/written communication and interpersonal skills; supervisory experience desired. Ability to manage multiple tasks and produce expected results; strong analytical and problem solving skills. Patient education experience preferred. Commitment to the mission of Care Alliance.


May function as clinic charge nurse. Triage, complete intake and basic health assessments and screening on patients according to Care Alliance protocols. Perform basic procedures, including venipunctures, IV's, injections, EKG's, and assist with other procedures. Maintain accurate health care records, maintaining confidentiality.


Registered Nurse with current Ohio licensure; BSN preferred. Minimum three years' experience; primary care, chronic care, outpatient, community health and/or pediatric nursing required; supervisory experience preferred. Previous experience with individuals who are underserved, homeless or living in public housing preferred.
