Job Description

The Benjamin Rose Institute on Aging is a premiere, not-for-profit organization in Cleveland, Ohio, and a nationally and internationally recognized leading organization in the fields of aging research and services. Our Home Care department is seeking a full time Nurse Case Manager (40 hrs). EOE.

Essential Areas of Responsibilities

  • Provision of nursing services to Home Care clients as assigned.

  • Provides quality nursing care including assessments on skilled and/or non-skilled cases.

  • Provides nursing procedures, interventions, and instruction/guidance to clients and/or families in skilled and/or non-skilled cases.

  • Keeps Director of Home Care fully informed about a client's condition and care provided and makes recommendations for alteration in the nursing and/or HHA care plan as needed.

  • Instructs and supervises HHAs and completes skills checks as required.

  • Orients/trains/mentors new nursing staff.

  • Documents clearly and accurately in client records in a timely basis per ESI/BRHC policy.

  • Contacts physicians, provider case managers and ESI staff as needed to report changes in client condition and in the case of skilled clients, to receive new medical orders.

  • Manages a non-skilled and/or skilled caseload under the direction of the Director of Home Care.

  • Maintains compliance with funders, including WRAAA, ODH, and Options.

Additional Areas of Responsibilities

  • May participate with Research in grant projects as requested

  • Performs Chart reviews and HHA audits quarterly and as needed.

  • Participates in Performance Improvement meetings and activities as directed

  • May act as Nursing Supervisor in absence of Director to manage daily operations and Admin staff.

  • Coordinates with Director, for on-call weekend and holiday coverage.

  • May participate in health and wellness screenings and presentations as requested.

  • Serves on BRIA committees as needed.

Supervision Responsibilities and Duties

  • Participates in interviewing, hiring, orientation and training of Home Health Aides.

  • Supervision of Home Health Aides as assigned.

  • Instructs and supervises Home Health Aides.

  • Assists in evaluation and competency checks of Home Health Aides.

Job Qualifications

  • RN Must have a current license to practice as a registered nurse in the State of Ohio.

  • One to two years' plus experience as a registered nurse is essential.

  • Prior experience in home health care and gerontological nursing care is strongly preferred.

  • This position requires a broad knowledge of nursing practice and health care delivery.

  • An insured car and a valid Ohio driver's license are required.

  • The vehicle insurance must meet the requirements and proof of financial responsibility per policy.

  • Background check and Drug test required.
