Job Description

Experienced, licensed social worker, counselor or nurse needed to provide Behavioral Health Crisis Services and assistance to individuals requiring support in the community of Lake County.

This is an exciting opportunity to be a part of a successful and growing team!

Essential Responsibilities

  • Provide and complete crisis assessments with individuals (in home, office or community) regarding level of danger to self/others, severity of psychiatric symptoms, and existing support systems to determine a recommendation of continued outpatient or inpatient care.
  • Develop crisis plans that include referral and linkages to appropriate services and coordination with other systems. The crisis plan should also address safety issues, follow-up instructions, alternative actions/steps to implement should the crisis reoccur, voluntary/involuntary procedures and the wishes/preferences of the individual and parent/guardian, as appropriate.
  • De-escalate and/or stabilize the individual and/or environment, linking the individual to the appropriate level of care and services including peer support, assuring safety, developing a crisis plan, providing information as appropriate to family/significant others, and resolving the emergent situation.
  • Develop and maintain strong relationships with community partners who provide support and care to individuals and families with behavioral health needs.
  • Coordinate services with community psychiatric support workers, prescribers and other service providers as needed, to aid in the behavioral health stability of consumers.
  • Coordinate services with emergency transport and local hospitals when necessary.
  • Provide advocacy, linkage and referrals.
  • Provide support calls to individuals seeking extra support with behavioral health symptoms.
  • Provide short term case management and counseling services as needed.
  • Conduct follow up visits with consumers recently discharged from inpatient hospitals and provide linkage to appropriate outpatient services.
  • Must be able to work independent and in a team.
  • Must be open and comfortable with responding to crisis situations in a calm, empathetic and flexible manner.

Qualifications, Education and Licensure

  • Bachelor's or Master's Degree in Social Work, Counseling or Nursing.
  • Previous experience in the mental health field is preferred.
  • Full Time position, some evening hours required.
