Experienced social worker, counselor or nurse needed to provide Case Management for

Community Based Crisis Services in Lake County

Essential Responsibilities

  • Assist and Support Crisis Clinicians while Clinicians complete face to face crisis assessments with individuals (in home, office or community) regarding level of danger to self/others, severity of psychiatric symptoms, and existing support systems to determine a recommendation of continued outpatient or inpatient care.
  • Assist in executing crisis plans that include referral and linkages to appropriate services and coordination with other systems. The crisis plan should also address safety issues, follow-up instructions, alternative actions/steps to implement should the crisis reoccur, voluntary/involuntary procedures and the wishes/preferences of the individual and parent/guardian, as appropriate.
  • Provide short term case management support and services, including advocacy, linkage and referrals for both consumers and their family members. The primary goal of services is to decrease the likelihood of the continuation of the crisis state.
  • Coordinate services with other community psychiatric support workers, prescribers, counselors and treatment team members as needed; to aid in the behavioral health stability of consumers.
  • Coordinate discharge planning for consumers leaving inpatient psychiatric facilities.
  • Conduct follow up phone calls and/or visits with consumers recently discharged from inpatient hospitals and provide linkage to appropriate outpatient services.
  • Develop and maintain strong relationships with community partners who provide support and care to individuals and families with behavioral health needs.
  • Provide Community Outreach and Education on available Crisis Services.
  • Provide support calls to individuals seeking extra support with behavioral health symptoms.
  • Must be able to work independent and in a team.
  • Must be open and comfortable with responding to crisis situations in a calm, empathetic and flexible manner.

Qualifications, Education and Licensure

  • Bachelor's Degree in Social Work, Counseling, Nursing or similar field.
  • LSW, LPC or LPN preferred.
  • Previous experience in the mental health field is necessary.
  • Full Time position, some evening hours required.
