Job Description

Position Description: The CPST Case Manager/Crisis Liaison provides support to children, adolescents and their families within the Lorain Youth Crisis Liaison Program. Through CPST, responsibilities include an ongoing assessment of client needs and ensuring linkages to natural and formal support systems in the community.

  • Provides Community Based Psychiatric Supportive Treatment (CPST) in the outpatient department and within the crisis program

  • Provides next day linkage and referral services through phone and community outreach post crisis assessment to ensure youth are connected to the appropriate follow up service

  • Provide cross systems coordination of care, including but not limited to, coordination with local juvenile court system, psychiatric hospitals, external child serving agencies and child welfare systems

  • Provide assistance to clients in achieving personal independence in managing basic needs

  • Facilitation of daily living skills

  • Collaborates with other disciplines and services to meet client needs.

  • Maintains a professional posture with clients and families that reflect courtesy, respect and the highest ethical standards.

  • Completes all required documentation completely, accurately, legibly and within specified timeframes.

  • Participates in training and learning collaborative' s to increase skills and develop competence in evidence based models of practice.

A Bachelor's Degree is required. An Ohio LSW or equivalent is preferred. A valid driver's license, vehicle and auto insurance coverage along with 1-3 years' experience in the mental health field, preferably with children, adolescents and their families. Strong organizational and time management skills, ability to work cooperatively as a team as well as independently, experience working with electronic medical records and ability to work effectively with diverse populations with complex psychiatric needs.
