Adventist Health Home Care Services brings quality care to home bound patients in the comfort of their own home throughout the greater Portland area. At Adventist Health, Home Health is available around the clock, including weekends and holidays. We know that patients facing a serious illness, or recuperating after a surgery generally prefer the comforts of home for their recovery. Our staff creates an individual plan of care for each patient, based on the patient's physician's orders to help manage care effectively with the utmost respect for the patient's dignity, privacy and well-being.

We're looking for someone to join our team as a RN (Hospice) who:

Delivers coordinated nursing care for patients in their place of residence according to established standards of care and the nursing process. Supervises and directs the activities of various levels of assigned nursing staff, and coordinates care with other disciplines while utilizing critical thinking, professional and supervisory discretion, and independent judgment.

Schedule: 5 pm to 8 am (combination of visits/office duties & on-call hours)*

* Must be available for orientation Monday to Friday (8 am to 5 pm)

Essential Functions:

* Collects relevant data pertinent to the patient's health or situation.
* Analyzes the assessment data in determining diagnosis and care issues.
* Develops a plan that prescribes interventions to attain outcomes.
* Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment.
* Evaluates progress toward attaining outcomes. Identifies outcomes for the patient or the patient's situation.
* Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment.
* Provides learning opportunities for patients/family members and team members.
* Directly provides health information to patients, families, and treatment team.
* Assesses and documents patient/family learning needs, implements teaching resources, and assesses and documents patient/family understanding.
* Completes all Oasis documentation accurately and completely.
* Delegates appropriately and coordinates duties of healthcare team members.
* Performs other job-related duties as assigned.

You'll be successful with the following qualifications:


* Associate's Degree in Nursing (ADN): Required
* Bachelor's Degree (BSN): Preferred

Work Experience:

* Acute care, home health, hospice or skilled nursing experience: Required

Required Licenses/Certifications:

* AHA Basic Life Support (BLS) certification: Required
* Valid Driver's License (DL): Required
* Registered Nurse (RN) license in the State of Oregon: Required
